Patient information

To secure a booking with us, all patients will be required to provide the following:

  • Full name
  • Address
  • Telephone number
  • Email address

Your information will not be shared with any third parties, nor will we send you unsolicited information.

Your information is protected and treated as confidential in accordance with current Data Protection legislation.

You can choose to remove yourself from our mailing list at any time.

Before and after pictures will be taken for all treatments and may be used for marketing unless otherwise agreed.

Appointments

All treatments require an initial face to face consultation during which patients will be asked to complete a comprehensive health questionnaire. Consultations will be carried out by your aesthetic clinician and prescriber Dorota Jaworska.

We reserve the right to decline treatment if the patient is not medically fit for the treatment or if it is in the patient’s best interests not to receive the treatment.

A booking fee of £40 is payable to secure your consultation or treatment appointment. This amount will be deducted from the total cost of any treatment you decide to go ahead with. The remaining balance is due on the day of treatment.

If an appointment is not cancelled with at least 48 hours notice, then the booking fee of £40 will be kept.

We offer a review appointment for dermal filler and botulinum toxin as part of our service; this review is carried out between 2-4 weeks after treatment and will not be offered after 4 weeks have passed.

Payment

A booking fee is payable for consultations and treatment bookings; this £50 fee is redeemable against any treatment carried out within one month of the consultation.

Full payment will be collected at the time of the treatments.

We accept payment by bank transfer or cash.

We are able to offer finance.

The pricing of treatments is subject to change at any time.

Refunds

Packages and treatments are to be paid in full before treatment commences and are non-refundable.

DJ Clinic will not provide refunds on treatments that have been provided in good faith or refunds on treatments if the patient has had an adverse reaction.

Children

We do not treat anyone under the age of 18

We would ask if we were not to bring children to your appointment, but we understand that if you do not have child care in place, they may need to attend with you.

Feedback

We always welcome feedback both, positive and negative, and will use this information to try and continually improve the service we provide.
